Parent Transportation Request
These transportation forms are designed for our department to ensure that we can provide your student the safest and most reliable way to get to and from school. Our transportation department follows school board and state policy which permits the applicability for your students' transportation.
1.Select the school your student will be attending (Early Learning, Washington, Schuler/Atlantic Middle School, Atlantic High school)
2. Fill a form out for each student requesting transportation
3. Fill every line on form including; Parent/guardian name, parent Email address, Address of pick-up/drop off location (1234 Main St. Atlantic IA 50022), Student Name, Grade, Male or Female, Phone number, Emergency Contact, Pickup/drop off days requesting.
